Parent feedback forms

Hi
I'm due my Ofsted inspection in the next 3 weeks and would like to show some parental feedback. Has anyone got a good questionnaire template that I can have a look at? I've got writers block and can't think what to ask! Many thanks

Ofsted will ask parents - if they manage to speak to them -

When did you last receive a report about your child's progress?

Does your cm share information for home learning with you?

Does your cm ask you about your child at home? ... things like that.

So those are the questions I ask parents to comment on :D
