Sharing resources feedback

Hi, would anybody be willing to share a copy of the form etc that they give to parents so that they can give feedback on resources I send home with children such as a counting or colour pack?

I don't use a form - our parents aren't the best with written feedback but they are always happy to talk to me about what's happening at home and send me photos to print out here :D

No form here, with just a few parents to deal with there is no point creating paperwork, could seem very formal and off-putting to parents too I think.
