Evaluation of activities

Do any of you do any written evaluation of planned activities? How well they went? Or what you would do differently next time? I am doing a Quality Improvement Award with my LA and one the things it asked was what is my process for carrying out planned observations. Is there any benefit to having a formal process? I evaluate how well activities go in my head as we go along and think about possible next steps.

I do it in my head. I work alone so no one to share it with, no manager to read it, so no point writing it. If you like to make yourself notes then of course you do what suits you, but no 'need' to write a written evaluation of an activity.

I mainly do it in my head, I might if I was going to repeat an activity write something on the plan, like add x y and z....
