Does anyone have a good and effective training evaluation sheet they can point me in the direction of? I can't find one online.

I know that Sarah had one in free resources (or possibly on her blog for independent childminders). I've looked but can't find it. These are the questions I ask myself (Sarah's plus some of my own, if I remember rightly):
Course / training / book review
Course –
Date –
• What did the course / training / book cover?
• What did I learn from the course / training / book?
• How will this benefit my work and outcomes for children?
• How will I implement what I have learned?
What further research do I need to do?
What resources can I buy or find or make?
