children evaluating activities?

Hiya guys! I evaluate my activitys I do with the children but how do I show ofsted that I let children evaluate their activities?
Does anyone have a sheet or info that they wouldnt mind sharing

I ask them and I often include their comments in my evaluations - just notes about what they think. Usually you can show it when you repeat activities - you wouldn't bother doing it again if they didn't enjoy it! :laughing::D
