does anyone use first aid book that has duplicated pages?

I use a MM one but it is not duplicated and recently when a child had an accident I literally filled it out a few minutes before mum arrived and hadn't had chance to get her to sign and copy it. She ended up taking child to hospital and I really feel she should have had a copy of it with her rather than when I saw her the week after Apart from using a bit of carbon paper does anyone have any suggestions or use a one that has duplicate pages.

I take a photo of the page after the parents have signed and email it straight over to them :)

I photocopy on my printer - once when I didn't have ink a parent took a photo on her phone :D

I like the idea of taking a photo - either so parent has copy on their phone or so it can be e-mailed.
My main concern was this child was quite accident prone and I could imagine the hospital having to be quite rigorous in their questioning of the parent it would have been so much easier for the parent to have the exact details of how/when /where it happened - and of course the fact that it wasn't even the parent that was with the child at the time.

I use mm ones and they are self duplicating forms, if they weren't I'd photocopy on my printer.
