need advice about my obs form please?

Hi I've attached my obs form...
Just thought someone can give me advise about "Playing and Exploring/Active Learning/Creating and Thinking Creatically" bit...
Do i need to tick it in every observation?
Should children show all of them?
Hope you will know what I mean...

Love the form, l would say no you don't always have to tick each section especially if its a short ob just the strongest and most consistent, your next steps for the same/similar activity could be from one of the sections that you didn't tick, i.e if you don't tick C&TC your next steps could be to encourage child to think how to use the activity in a different way.

Well laid out, clear and easy to understand :thumbsup:
As far as ticking boxes...not all need to be ticked and whatever area hasn't been ticked could be used in planning for your next steps.
