Quick Question - PACEY Accident Forms

Hi,

Instead of using my own accident forms I decided to buy the folder From PACEY. Anyway most of the forms are fine but I am a bit confused with the forms for if a child comes to you with an existing injury.

My old forms asked how it happened, what they did, where it happened but these forms only ask for child's name, what happened and for a signature. Is this enough? Also it asks for time its filled in does anyone know why we need that?

I just write little existing injuries in the child's diary as 'handover notes' - big ones like head bumps go on an accident and injury form that just notes what is wrong and what parents tell me happened.
