What forms would you fill out for a child who has patches of red blotches that comes and goes?

Parent has taken to GP and health visitor who both said moisturise skin with an emollient as often as possible. Cream is not a prescribed

If GP says to moisturise they must have an idea of what it is? Eczema, possible allergy, etc.

I'm not sure how I'd document it, sorry.

I would write up the cream on one side of the form as a repeat medication - ask parents to sign to give permission for repeat administration of cream and to confirm times through the day.

Then do a table for the other side that fits dates and times plus my signature and parents initials at the end of the day to show they know what I have done.

Maybe 3 monthly reviews as parents wishes might change.

If it's eczema they would have cream such as diprobase which can be put on any time - but should also see a specialist eczema nurse for further advice - they might need to push for a meeting but it's invaluable :D

I'd document it as eczema, do as said with the form and moisturise as necessary.

Eczema comes in all forms of rash, red, itchy types. My son has it, I have it, and it looks completely different.

It is a cream similar to Diprobase Sarah and is to be applied anytime just as often as we can. I am using pacey forms would you put it on permission to administer medicine over extended period of time/continuous treatment?
