Leavers Questionnaires

Does anyone give out questionnaires to LO's/parents who are leaving?

I have one LO leaving in a few weeks to go to a school nursery and wanted to get some kind of feedback from parents about LO's time here but have no idea where to start, any help would be greatly appreciated!
