Parent evaluation form

Hello, I think it's about time I got the parents to fill out an evaluation form, any suggestions of questions I can put on it ????? Thanks x

You need to ask questions like:
Are you happy with the care I give?
Is your child happy at my setting?
Is there anything your child does/doesn't enjoy doing at my setting?
Do you have any concerns over your child's development?
Are there any aspects if your child's care you think I could improve on?
Any other comments?

Hi, I was about to update my menu at he same time so asked are thy happy with meals provided, any suggestions etc, and were they happy with the means of immunisation; brief chat, daily diary etc x
