Is it literally going aroundy house and writing out possible risks and the writing what I've done to minimise the risk ??

Yes that pretty much it.
Go around each room being used and write everything down from a missing toy part to a curled up rig for example.
Then list the action taken or if not what you will be doing if its not solverble straight away. For example a loose fence in the garden, then tape of that section and log the details of who you have contacted to come out and fix it and when.
