Dear XX,

It is with regret that I am writing to inform you of my intention to terminate our agreement to provide childcare for XX at the end of the 2012/2013 academic year, as a result of a change in our family circumstances. X’s last day within the setting will be Tuesday 23rd July 2013.

Should you require assistance in finding another childminder then I would suggest contacting school or the Family Information Service on xxxxx.

I would say "contract" rather than "agreement", but otherwise it seems good to me!

I always remind parents that if they get tax credits it is their responsibility, not mine, to inform them - and give the tC phone number :D
