under the new eyfs you do need to offer a chance for them to feedback to you and to offer them development opportunities hon.
There are lots of simple formats around for appraisal. From my prior HR background the best advice I can give is:
ask them what they think they did well
what they think they could've done better
what they would like to develop/have help with/training on
any concerns
then, you tell them:
what you think they did well
what you think they could've done better
what you think they need support with
together hatch an action plan :)
