Permission for sharing information with nursery

Does anyone have an example of a permission form for sharing information with a child's nursery.
One of my LOs is starting 2 days a week in nursery after Christmas and will still be with me one day a week.
I have written a letter introducing myself to them and asking them to share information but want to include something which parents can sign to say they give permission to share information.
I'm hoping nursery will be able to share info by email as I will not be taking her there or picking her up, so doubt I will ever see them otherwise.

hi, have attached mine for you.

i also make sure i have a contact number /email for preschool and give them my contact details ( probably should put them on the form! )

i either ask them to sign 3 copies (one each) or i copy it and give a copy to each of us!
