Parent Questionnaire

Hi guys,
I have now been up and running for 1 year. It's gone sooo fast!
Anyway i was wondering if anyone could give me some ideas of what to put in a questionnaire for my parents. I would like to find out what they think i am doing well or not etc..
I am sure a lot of you already do these so would love any ideas or help that you can give me. :)

i had a very 'in depth' questionaire to start with ( think it was to do with my quality first ) and one parent went through with a fine tooth comb, didn't say a positive word and kept writing ' i presume so, no evidence shown' to things like is childrens art work valued and displayed ( they obviously have their eyes shut all the time ), does childminder practice equal ops etc

i now use the bromley one and find it gets answers and parents can be as short or as long with their answers as they like :D

For a very simple questionnaire I ask for three things they like about my setting and three things that I could add or improve
