i have just asked the doctors for a price on this and it could be between £15-£25 which sounds ok to me but this is only if its a short form to fill in

more if longer :(

i have not got my yet as im just about to register with ofsted but can anyone tell me how long these ones are so i can budget the form for my expenses

you fill in the majority of the form, the Dr's bit is quit small, just point it out to the recpetionist when you give it to her, as the frm looks heafty, but the majority is filled out by you :)

Hi,you fill your bit in and sign it .Then the dr checks it and signes and dates it. My dr charged me £50 for mine.What a rip off:mad:

that is a rip off but i suppose we cant moan too much we need it !!

I was charged £87.50 - this is the amount that the British Medical Association suggests that GPs charge :(

The receptionist at my surgery said that it would cost about £15, but the GP obviously wanted more :rolleyes:

Gosh that's a lot, mine was £35.00 and considering there was nothing on it so it would have taken all of 5 mins to fill in it's not a bad earner!! xx

Tell me about it! I wouldn't mind if i had a complicated medical history but i'm rarely ill, i can count the number of doctors visits ive had there on one hand. :rolleyes:

I'm just going through the process of applying to ofsted now. Where do I get the form? From the DR or does Ofsted give you the form?

Any help appreciated!!

Ok, I have downloaded and completed my part of the health declaration form. It says to put it in a pre-pain envelope for your Dr to post off once completed. Does anyone have address??

Not sure on the address, sorry, but my doctor sent my form off (£35) before I sent in my other forms so Ofsted sent my medical form back to me, very confusing so rang them and they explained when they received my medical form they did not know who I was as the other forms crossed in the post :panic: so had to send medical form back again.
I went on the Ofsted site for the address.

Doctors charge was £20 for him filling in the form and it cost £15 for the receptionist to file on the computer and post :eek:

I gave mine to the receptionist at my doctors and she said it would be about £15 but when phoned to ask if it had been done she said yeah just need to pay the bill of £10 which i'm quite chuffed with so believe it depends on your doctors surgery

Both times ive registered I have not been charged at all for this!:thumbsup:

Some of the amounts people have had to pay on here is ridiculous

I paid £25 for mine last year, was outraged at that because all the doctor had to do was tick a box and sign her name!

I had to pay £30 and it took my doctor 3 weeks to fill in - grrrrrr
