I found a link on here yesterday, which took you to the government/cssiw/publications site - and it showed a blank SAF and the last page was a document giving pointers to think about when answering each question.
I cant find it now for love nor money - has anyone seen it?

MS WG is in wales so its a SAF not SEF for english childminders :D
